Specialized Adult Day Care Program
Do you or your loved one have Alzheimer's or another form of
dementia? Visitng Nurse Specialized Adult Day program is designed to provide
assistance for adults with Alzheimer's disease or other related dementias
in need of supervision and social interaction. The program offers respite
care for the family in a safe, structured setting.
Our Specialized Adult Day Care program is designed
to provide:
- Mentally and physically stimulating activities which are adapted to the
physical and cognitive functioning level of each individual.
- Mental and physical stimulation to maintain physical and cognitive functioning.
- Social and recreational activities.
- Caring, comfortable, home-like supervised setting.
- A staff of specially trained registered nurses, medical social worker,
home care aides and volunteers.
- Nutritious morning and afternoon snacks.
- Council On Aging hot meals (available at cost).

Family Services:
- 24 hour crisis line staffed by a Registered Nurse

Eligibility
- St. Clair county resident
- Diagnosis of Alzheimer's disease or other related forms of decreased mental
abilities (dementia)
- Must be ambulatory - walker okay
- Living at home with family member
Fee
Assessed on a sliding fee scale based on patient's taxable income.

9:00 A.M. to 3:00 P.M.
24-HOUR ON-CALL SERVICE
FOR AFTER 5:00 P.M. AND WEEKENDS
